There are new (blue) Charter Forms on the TfL webstie at
https://tube.tfl.gov.uk/charter/charter.pdf with space for Oystercard
details.
I'd prefer they got the online from at
https://tube.tfl.gov.uk/secure/charter/
working again - it's been
unavailable for ages.
