Default Oyster at train/tram interchanges

Does anyone have a full list of what one has to do with Oyster PAYG to
correctly interchange between the train and tram at each station? I know
that at some the tram is outside the station but others look complicated,
particularly the less well known stations.

This web site
http://www.oyster-rail.org.uk/
has everything you need to know about interchanging with Oyster, including
an entire page on Wimbledon.

It looks good and Wimbledon now seems clear. A few of the other stations do
still confuse. If I've got this right:

Tram to train - always touch in on a yellow reader at every station.

Train to tram:

West Croydon, East Croydon & Beckenham Junction - always touch out leaving
the train station and touch in at the tram stop.

Wimbledon - always touch in on the readers on platform 10; they'll sort out
any other touch in within the station.

Elmers End - touch in on the tram readers on the tram platform only? It's an
island shared with the train and the only time I've used it there wasn't
clear signage for those in a hurry to know which is which?

Birkbeck - Does this work like the Croydons with an out and in or like
Wimbledon with only the tram reader needed?

Mitcham Junction - I've never interchanged here so have no idea if there's a
clear gap between trainland and tramland or if it's spot the green sticker
time.

Which of these did I get wrong?
