Default Contractors drill through tunnel ceiling - again ...

No excuse for the contractors that it was a non LU-tunnel - the Old
Street area breach was onto the Northern City line, which is Network
Rail's responsibility to safeguard - iirc the contractors had only
checked their plans with LU (something like that).
