What's the difference on the tube map between stations with step-free
access to trains versus step-free access to platforms? Surely the
trains call at platform level?

Not really; the gap between the train and the platform can be
substantial (enough to make access in a wheelchair challenging).

Knowing the size of the gap - both in terms of the horizontal and
vertical distance - can be important to some folk. Luckily one
of the access guides gives you exactly that info.
