Default Metropolitan District Railway Early Stock

I'm working on an online history of UK electric multiple units and currently
I am concentrating on the early stock of the MDR.

I am wondering if anyone can point me to references that will help me with a
number of questions I have.

1. Two of the A Stock trailers were converted to control trailers. I am
aware of the identity of one of these and I am trying to identify the
second.

2. 3 B stock driving motor cars were written off after accidents and I am
looking for more details.
