Default Historical Underground map

There is a Historical Underground map at Rickmansworth station, showing
the
dates that all line segments and stations opened & closed, as well as
station name changes (including Chorley Wood - Chorleywood !).

Does anyone know where a copy of this map can be obtained (either hard
copy
or download) ?

Available to buy from most transport-related bookshops in London, and also
at Amazon...

Never come across an online version - and the full thing unfolds to quite a
size, so I'm not prepared to even attempt to scan it in - even without the
copyright issues....
