I suggest the Bakerloo Line from Paddington to Baker Street, then a level interchange to the Jubilee Line which will take you to Southwark. The journey itself takes about an hour. About 10-15 minutes longer but much cheaper is an all-Tube route using the Piccadilly Line from Heathrow. The terminal your plane arrives at, the day of the week and time of day, and the precise address (e.g. postcode) in Southwark could all affect this advice.
