Hammersmith, Paddington, Edgware Road, and Canary Wharf spring to mind as cases where you can't get from one to the other inside the gateline (or am i wrong about Canary Wharf?). There are enough stations with merely sub-optimal interchange that i'm not going to start listing them ... You are perfectly right about Canary Wharf. It is even a slightly shorter walk between Canary Wharf Jubilee Line and Heron Quays DLR than between Canary Wharf Jubilee Line and Canaray Wharf DLR... Yes, i remember that being the case.
