Default 'Near miss' between District and Piccadilly line trains near EalingBdwy

Incident happened last Friday (27 March) - appears to have been
precipitated by the District line driver 'SPAD-ing' after leaving
Ealing Broadway, though thereafter he is said to have acted "by the
book". A signalman has been relieved of duty whilst the investigation
takes place.

BBC News suggests the driver did act by the book in that he had obtained
permission to proceed after the SPAD only to find another train crossing his
path. Do they keep recordings of the train-to-signalbox voice communications
(like they to with ATC)?

The RAIB have, somewhat belatedly, announced today that they are
investigating this incident.
