Could it be they didn't sell a boundary ticket but a ticket from that
station instead and the fare is calculated differently?

They should be able to do it at Farringdon as they are used to NR fares...

I can assure you if a tube inspector stops someone who says his Oyster card
has been stolen they will check it out. Yep it may involve some note taking

You can but really only where they have the readers. They are discouraged
at other stations. Also when it comes to pre-pay National rail aren't set
up for it.

Oyster cards have:

3 Ticket slots (TKT)
3 Pre-pay slots (PPY)

What Dave says is almost correct except pre-pay has its own slots.
